1. In order to get the criminal record check extract from Ukraine go to the link: https://vytiah.mvs.gov.ua.

2. The extract made in electronic form will be certified by the electronic seal and sent to the e-mail address specified by the applicant.

 If needed, the Consulate will be in position to issue the respective document after the required data has been received by the applicant from Ukraine’s competent authorities.

 For certified document from the Consulate:

1. Create an appointment https://online.mfa.gov.ua/.

2. A valid passport is required as an ID proof and should be presented on the day of application.

3. Consular fee is $30 in a form of Money Order.

To obtain a Paper Extract, the Applicant must personally submit a paper request to the Ministry of Internal Affairs of Ukraine, present an identity document with a copy.
